Recommended outline for case reports

Requirements:

1. 800-1000 words for each case
2. 3-5 references
3. Structure as outlined below:
    1) Identification
        • Name (initial only)
        • Clinical record number
        • Hospital / outpatient clinic / other
    2) Initial Visit
        • Clinical history
        • Finding of physical examination
        • Investigations requested (lab – others)
        • Initial diagnosis
        • Initial treatment
    3) Follow Up
        • Clinical evolution
        • Physical examination changes
        • Lab and others exam result
        • Final diagnosis
        • Treatment
    4) Discussion
        • Fundaments of Diagnosis
        • Differential diagnosis
    5) Type and Form of Candidate Participation (brief description)

Label each case with Part I case 1, Part I case 2 and so forth; and Part II case 1, Part II case 2 and so forth.
Can send the 10 Part I cases in one file or separately in 10 files but label as suggested above. One file is preferred. The same applies to 10 Part II cases.
Clinical photos are not necessary.
